Students must submit a typed description of the extenuating circumstances that lead to this requirement, as well as justification and a plan for the future. Depending on circumstances, additional supporting documentation may be required for all requests. This includes medical, faculty or institutional support. Please be aware the report must be on office letterhead and include reasonable explanations and/or justifications validating your statement. All documentation will be kept confidential.
